Accessing the German-Canadian Care Home

How do I access the German-Canadian Care Home if I am living at home?
The first step is to have a health care needs assessment completed by a Home Health Case Manager. This assessment can be arranged by contacting your family doctor or your local health unit. The Home Health Case Manager will determine your needs and eligibility. Priority is always given to those with the highest need and at the greatest risk.

What happens if I need to access residential care services from hospital?
If your are assessed as needing residential care following a stay in the hospital, the hospital social worker will work with you and your family and arrange for either
- a transfer to a residential care facility from hospital,
- a return home with appropriate care and services in place while waiting for a suitable publicly subsidized residential care bed, or
- a move into a private pay residential care is while waiting for a publicly subsidized residential care bed.
How long does it take to access the German-Canadian Care Home?
Once it has been determined that you are eligible for a subsidized residential care home your name will go on the waitlist with Vancouver Coastal Health (VCH) Priority Access and the search for a bed begins immediately. You will be able to indicate your place of choice. While Priority Access will do everything to accommodate your preference you may be offered a bed in a different facility first. If you accept the bed your name will go on a transfer list for the German-Canadian Care Home and you will be offered a bed in our Home once it becomes available. It is difficult to predict how long this will take as waiting periods vary.
How should I prepare to move into the German-Canadian Care Home?
We recommend that you book a tour of our home. Please contact our business office assistant at 604-713-6510. The time between being notified of an available bed and moving into the German-Canadian Care Home is often very short. We strongly recommend that you organize and prepare yourself for the move in advance.
