The care team is comprised of a variety of health care disciplines such as nurses, care aides, recreation therapist and aides, music therapist, dietitian and rehab staff. They are dedicated to helping each resident maintain maximum independence and well-being. The resident and family are encouraged to be part of this team and to participate in the planning of the resident’s care.
The interdisciplinary care team completes a physical assessment on each new resident within the first few weeks of admission. Within six weeks of admission an interdisciplinary conference is then held at the care home. The care team, physician, pharmacist, family and, if possible, the resident participate in the care conference. It offers an opportunity for residents and families to meet care team members, to discuss issues of concern, and to participate in the planning of the resident’s individualized care. Care conferences may be held at any time but at least on an annual basis.
