Rehabilitation Services

The German-Canadian Care Home has an in-house rehabilitation department. A team of occupational therapists, physical therapist and rehabilitation assistants delivers carefully designed programs that use the coordinated efforts of an interdisciplinary team to contribute to each resident’s rehabilitation. Their therapeutic programs help residents improve mental and physical well-being and contribute to quality of life. Services are provided five days a week.

Each resident will receive an initial assessment which is the foundation of an individualized rehabilitation program. The goals of the program are to maintain and improve residents’ physical and functional abilities.

Rehabilitation programs include self care skills that encourage independence to dress self, complete hygiene tasks and feed self, mobility skills, transfer training, fall prevention, seating and positioning for appropriate wheelchair and adaptive equipment, appropriate mattresses that promote best mobility, comfort and skin protection, pain management and dysphagia/swallowing assessments.

In addition, the team has developed programs to promote cognitive skills such as a women’s group and a men’s group that meet weekly for cognitive stimulation.

The Occupational Therapist participates in the care conferences and monthly unit conferences.
