Rehab Program
The German Canadian Care Home offers therapeutic rehabilitation services to its residents as part of their
resident care program. Services are provided in house by professional occupational therapists, physiotherapist and rehabilitation assistants.
During the first weeks following admission rehab staff will conduct assessments and develop and an individualized rehabilitation program for each resident. The goals of the program are to maintain and improve residents’ physical and functional abilities. Our rehabilitation program includes support with Self Care Skills (dress self, complete hygiene tasks, feed self), Mobility Skills and Transfer Training, Fall Prevention, Exercise Programs, Seating and Positioning, Pain Management And Spasticioty Management and Dysphagia/Swallowing Assessments, Cognitive Stimulation Exercises and Socialization Skills.
Each resident will be reassessed at least once a year or more often if needed. Rehab staff is part of our care team. The occupational therapist will participate weekly in the resident care conference, where annually, each resident status and evolution is reviewed by the Registered Nurse, Physician, Pharmacist, Nutritionist, Occupational Therapist and Recreational Therapist.
Rehab staff can assist families and residents when selecting a wheelchair and other equipment.
