We have a dynamic health care team that includes: nurses; care aides; occupational therapists; a physiotherapist; a Quality of Life Manager; a recreational therapist and her assistants; a music therapist; a pastor; a dietitian and an art therapist. They bring their diverse skills, education and talents to support resident independence and well-being.
Upon moving into the Home several assessments and conversations take place that help get the resident, family and care team acquainted. The first resident conference is scheduled approximately six weeks following arrival. This is an opportunity for everyone involved to engage in a dialogue regarding the well-being of the resident. Other such conferences can be scheduled as the need arises.
The team also includes a Medical Director, Dr. Aileen Iloreta, who provides medical leadership to other attending physicians and the care team. Dr. Iloreta attends most resident conferences, sits on our medical advisory committee, provides education to staff, and oversees clinical risk management and quality improvement processes.
