THE CARE TEAM

The care team is comprised of a variety of health care disciplines that are dedicated to helping each resident maintain maximum independence and well-being.
The resident and family are encouraged to be part of this team and to participate in the planning of the resident’s care.
The interdisciplinary care team completes a physical assessment on each new resident within the first few weeks of admission. An interdisciplinary conference is then held after the resident has been residing within the  facility for six weeks. Care conferences are intended to provide the opportunity for residents and families to meet care team members, to discuss issues of concern, and to participate in the planning of the resident’s individualized care. Care conferences may be held at any time but at least on an annual basis.
